Penile Prosthesis Implantation

A penile prosthesis is an implant placed inside the cavernous bodies. It replaces the mechanism of erection when the body can no longer provide it: the patient controls rigidity himself, and intercourse becomes possible again regardless of the cause of the dysfunction.

The surgery is considered when tablets, injections and other conservative treatment no longer help. Professor Kurbatov selects the type of prosthesis after examination — the models differ in how they work, in the sensations they give and in cost.

At a glance

Indication

Implant types
semi-rigid and three-piece hydraulic; two-piece models are practically no longer used
Preserved
sensitivity of the glans, orgasm and ejaculation
Reversibility
irreversible: the implant replaces the cavernous tissue
Sexual activity
after the doctor confirms healing and activation; timing is individual
Combination
possible together with Professor Kurbatov's enlargement techniques

The exact treatment plan is determined by the surgeon at an in-person consultation.

Doctor of Medical Sciences, urologist-andrologist, specialist in reconstructive genital surgery

“A prosthesis is considered when medication and injections no longer work. It is not a compromise but a solution with a predictable result: erection becomes controllable again. Which model suits a particular patient is decided after examination — the anatomy, the manual dexterity of the patient and his expectations all matter.”

When Prosthesis Implantation Is Considered

The common feature of all these situations is that the erection cannot be restored by medication.

Erectile Dysfunction Resistant to Treatment

Tablets do not work or are contraindicated, injections into the cavernous bodies no longer give a sufficient result.

After Radical Prostatectomy

Damage to the nerve bundles during surgery for prostate cancer is one of the most common reasons for implantation.

Consequences of Priapism and Vascular Damage

Prolonged priapism, severe diabetes and vascular disease lead to fibrosis of the cavernous tissue, after which erection does not return on its own.

Peyronie's Disease with Erectile Dysfunction

When curvature is combined with dysfunction, the prosthesis is implanted together with corporoplasty — both problems are solved in one operation.

About Peyronie's disease

After Unsuccessful Interventions

Consequences of previous operations and of substances injected into the penis are assessed separately: reconstruction may be required before implantation.

When the Decision Is Made Consciously

Implantation is irreversible: the prosthesis replaces the cavernous tissue and a return to a natural erection is no longer possible. This is discussed in detail before the operation.

Types of Prostheses

The models differ in how rigidity is controlled. The choice depends on anatomy, hand function, lifestyle and budget.

Semi-rigid (plastic)

Two flexible rods inside the cavernous bodies. The penis keeps constant rigidity and is bent up or down by hand. The simplest design: nothing to break, control requires no effort.

Three-piece hydraulic

The cylinders are placed in the cavernous bodies, the pump in the scrotum, and the reservoir with sterile fluid behind the pubic bone near the bladder. The closest to a natural erection: outside intercourse the penis remains soft, rigidity appears only when the pump is used. The most complex design and the highest cost.

Two-piece models are hardly used any more, so the choice in practice comes down to these two designs.

Which model is available and which is recommended in a particular case is discussed at the consultation: the difference between designs is felt in daily life, not only during intercourse.

How the Operation Goes

Implantation is performed under anaesthesia chosen together with the anaesthesiologist. Access is through a small incision at the base of the penis or in the scrotal area; the cavernous bodies are prepared and the cylinders are placed inside them. In hydraulic models the pump is positioned in the scrotum and the reservoir in the lower abdomen.

Infection is the main risk of any implant surgery, so antibiotic prophylaxis, sterile technique and careful tissue handling are part of the protocol rather than an option.

Hospital stay is short. The exact duration of the operation and of the stay depends on the model and on the condition of the cavernous tissue — in cases of fibrosis the surgery takes longer.

Recovery

The prosthesis is not activated immediately: the tissues must heal around the implant first.

First days

Swelling and discomfort are expected. Dressings, antibiotics and limited physical activity.

Healing

Sutures and follow-up appointments. During this period the prosthesis is not used, and the surgeon monitors how the tissues respond to the implant.

Activation and training

The surgeon shows how to use the pump. The first attempts are made in the clinic — it is a skill, and it takes a little practice.

Return to intimacy

Sexual activity is allowed after the doctor has confirmed healing and activation. Exact dates are set individually.

Complications and How They Are Prevented

A prosthesis is a device inside the body. Honest conversation about the risks is part of the preparation.

Infection

The most serious complication: an infected implant usually has to be removed. Prevented by antibiotic prophylaxis, sterile technique and preoperative screening; diabetes control matters too.

Erosion

Pressure of the implant on the tissues. Prevented by correct sizing of the cylinders — one of the reasons why measurements are taken during the operation.

Mechanical failure

Any device has a service life. Modern hydraulic models are reliable, but a malfunction over the years is possible — it is resolved by replacing the prosthesis.

Revision surgery

Replacement or repositioning of the implant. Technically more demanding than the primary operation, which is why the first surgery should be done properly.

Prosthesis Combined with Enlargement

Implantation restores erection but does not increase the size of the organ; moreover, patients often notice a shortening after the operation. Professor Kurbatov combines prosthesis implantation with his own enlargement techniques, so that restoring function and correcting size are solved together rather than one after the other.

Whether the combination is possible in a particular case depends on the anatomy and the condition of the tissues — this is decided at the consultation.

Questions and Answers

Will the erection feel natural after penile prosthesis surgery?

After penile prosthesis surgery, rigidity is provided by the implant, so the mechanism differs from a natural erection.

Sensitivity of the glans, orgasm and ejaculation are preserved — the prosthesis does not affect the nerves responsible for them.

Is penile prosthesis implantation reversible?

No, penile prosthesis implantation is irreversible: the implant replaces the cavernous tissue, and after removal a natural erection does not return.

This is why implantation is considered only when other treatment no longer works.

How long does a penile prosthesis last?

Modern penile prostheses are designed for many years of use, and semi-rigid ones have nothing to break in them.

A mechanical failure after years is possible and is resolved by replacing the prosthesis — the service life of a specific model is discussed at the consultation.

Is a penile prosthesis noticeable to others?

In everyday life a penile prosthesis is not noticeable: the implant is entirely inside the body, there are no external parts.

With hydraulic models the penis stays soft outside intercourse, which makes them the closest to the usual appearance.

Will the penis become shorter after penile prosthesis surgery?

Patients often notice a certain shortening after penile prosthesis implantation — this is a known effect of the operation.

Professor Kurbatov combines implantation with his own enlargement techniques; whether this is possible in a particular case is decided after examination.

What examination is needed before penile prosthesis surgery?

Before penile prosthesis surgery: assessment of erectile function and of the condition of the cavernous tissue, ultrasound, plus the standard preoperative work-up.

Control of diabetes and exclusion of any infection are especially important: infection is the main risk for any implant.

Can a penile prosthesis be implanted after prostate surgery?

Yes, prostate surgery is one of the most common reasons for implantation.

The timing depends on how the recovery after the prostatectomy is going and is determined individually.
