Reconstructive Penile Surgery

Reconstruction restores the form and function of the penis after injected substances, injuries, scarring and unsuccessful operations — including those performed elsewhere. This is not aesthetic surgery: the goal is a working organ and a normal life, not an ideal picture.

Professor Kurbatov is a specialist in reconstructive genital surgery and a board member of the EAU section of urogenital and reconstructive surgery. The plan of treatment is drawn up after an in-person examination — such cases are never identical.

At a glance

What is treated
oleogranuloma (injected Vaseline), consequences of fillers and gels, of unsuccessful operations, injuries and burns; buried penis
Priority
a working organ — erection, urination, sexual activity; appearance comes after function
Stages
treatment may be split into two stages when the skin cover has to be restored
Oleogranuloma
it is dangerous to postpone: the inflammation is chronic and the volume of future surgery grows
Plan and price
only after an in-person examination, when the volume of surgery is clear

The exact treatment plan is determined by the surgeon at an in-person consultation.

Doctor of Medical Sciences, urologist-andrologist, specialist in reconstructive genital surgery

“Men come here after injections of substances that were never meant to be there, after other surgeons' operations, after injuries. Almost all of them expect to be judged. There is nothing to judge: these situations are common, and most of them can be corrected — the question is how many stages it will take.”

What Patients Come With

Four groups of cases. Each is described below — follow the link to the one that matches your situation.

Oleogranuloma (vaselinoma) of the Penis

A consequence of self-injection of vaseline or other oily substances under the skin of the penis.

The substance is not absorbed. It provokes chronic inflammation: the skin thickens and hardens, ulcers and fistulas appear, the surrounding tissue turns into scar. Over time this interferes with erection and with intercourse, and the process does not stop on its own.

Conservative treatment does not work here — the substance has to be removed together with the altered tissue. The extent of the operation depends on how much was injected and how long ago; when the skin cannot be preserved, the defect is closed with plastic surgery, sometimes in two stages.

The earlier a patient comes, the smaller the volume of surgery. Waiting makes the situation worse, not better — this is the main thing worth knowing about this diagnosis.

Complications After Fillers and Gels

Injection enlargement is sold as a procedure without surgery. Its consequences are treated surgically.

Gels and fillers migrate, form lumps and uneven thickening, and provoke inflammation and scarring. Unlike vaseline the damage is often less extensive, but the mechanism is the same: a foreign substance in tissues that were not designed for it.

Treatment means removing the substance and the altered tissue around it and restoring the contour. What can be removed completely and what will leave traces is assessed at the examination — a lot depends on the type of substance and on how long it has been there.

A surgical alternative to injections is thickening with a soft silicone implant: the implant is removable and does not spread through the tissues. About the thickening surgery

Correction of the Consequences of Previous Operations

Including operations performed by other surgeons.

Deformity after enlargement surgery, a displaced or palpable implant, excessive scarring, an unsatisfactory result of ligament transection — all of this can be reviewed. Repeat surgery is technically harder than the first one: the tissues are already altered, and the margin for error is smaller.

Therefore the conversation starts not with the technique but with what can realistically be achieved. Sometimes the answer is «in two stages», sometimes «the result will be better, but not perfect» — this is discussed openly before, not after.

When erectile function is lost as well, reconstruction is planned together with prosthesis implantation. About prosthesis surgery

Injuries, Scarring, Buried Penis

Post-traumatic deformities

Consequences of injuries and burns: scar contractures, curvature, skin defects. Restoring the skin cover is often the first stage.

Buried penis

The organ is hidden by excess pubic fat and skin. Fixation of the tissues and lipectomy of the pubic area make the visible length match the real one.

Skin defects and lymphoedema

Excision of altered tissue and plastic closure of the defect; the technique depends on the area involved.

How the Situation Is Assessed

An in-person examination is required. In reconstruction the plan depends on what the tissues feel like, how mobile the skin is, where the borders of the altered area run — none of this can be seen in a photograph sent by messenger, and no serious surgeon will quote a price from one.

Ultrasound is used to assess the depth of the changes, and photo documentation to compare the result afterwards. Only after that does it become clear whether the case is one-stage or two-stage, and what the realistic outcome is.

Stages and What to Expect

One or two stages

When the skin cover has to be restored, the treatment may be split into two stages: first the altered tissue is removed, then the defect is closed. The interval between stages is set by the surgeon.

Scars

Any reconstruction leaves traces. They are placed so as to be as inconspicuous as possible, but a promise of no scars at all would be a lie.

Function first

The priority is a working organ: erection, urination, intercourse. Appearance is dealt with once the function is restored.

Honest forecast

The expected result is discussed before the operation and in specific terms — including what will not be achieved.

Questions and Answers

Can injected Vaseline or gel be removed from the penis completely?

Vaseline and gels are removed together with the tissue they have altered, so the volume of surgery depends on how much was injected and how long ago.

What exactly can be removed in a particular case is assessed at the in-person examination.

Will scars remain after reconstructive penile surgery?

Any reconstruction leaves traces — a promise of none would be untrue.

Incisions are planned so that the scars are as inconspicuous as possible, and function always takes priority over appearance.

Why is penile reconstruction sometimes done in two stages?

When the skin cover has to be restored, the altered tissue is removed first and the defect is closed later, once the tissues have settled.

Not every case needs this: many are completed in one operation.

Does Professor Kurbatov correct the consequences of other surgeons' operations?

Yes, correcting the consequences of other surgeons' operations is a routine part of reconstructive practice.

Repeat surgery is technically harder, so the realistic outcome is discussed in detail before the operation.

Can the price of reconstructive surgery be given from a photo?

No, the price of reconstruction cannot be given from a photo: the plan depends on how the tissues feel, how mobile the skin is and where the borders of the altered area run — none of this is visible in a photograph.

The cost is named after the examination, when the volume of surgery is clear.

Is it dangerous to postpone surgery for oleogranuloma?

Yes: with oleogranuloma the inflammation is chronic, the tissue keeps turning into scar, and the volume of future surgery grows.

The earlier the treatment starts, the more tissue can be preserved.

Will erection be restored after penile reconstruction?

It depends on the cause: reconstruction restores the form and the skin cover, but it does not treat erectile dysfunction.

When erection is lost, implantation of a prosthesis is discussed — sometimes as a second stage of the same treatment plan.
