Ligamentotomy: How Penis Lengthening Surgery Is Performed

Ligamentotomy is a penis lengthening surgery in which the suspensory ligament of the cavernous bodies is divided. Professor Dmitry Gennadievich Kurbatov uses a modified technique that produces elongation not only at rest but also in erection. What ligamentotomy is

Part of the penis is hidden under the pubis and held by the ligamentous apparatus. The principle of the surgery is to divide the suspensory ligament of the cavernous bodies, followed by skin plasty with deepening of the skin angle at the base of the penis — this brings the hidden part of the organ outward.

How the surgery is performed

The surgery includes division of the suspensory and fundiform ligaments through an approach at the base of the penis. After the ligament is divided, a closed cavity forms — the professor recommends filling it with material to prevent the cavernous bodies from reattaching to the pubic bone.

A mandatory step is skin plasty with deepening of the angle between the base of the penis and the pubis, with fixation of the tissues to the pubic bone. This achieves permanent elongation both at rest and in erection in the range of 1.5–3 cm; the specific result depends on anatomy. With this technique, the change in the erection angle and the tilt of the organ is minor.

Use of an extender or stretcher in the postoperative period is not required with this technique.

Other lengthening techniques

Modified ligamentotomy is the primary but not the only technique. Professor Kurbatov also uses:

• Suprapubic lipectomy — an additional step when the suprapubic area is pronounced.
• Sliding plasty (the Brazilian technique) — a complex reconstructive technique used in selected cases.
• Lengthening corporoplasty with semi-rigid prostheses — a solution when several clinical tasks are combined.
• Lengthening corporoplasty with grafting — used by indication in more complex cases.

Some methods serve as the primary lengthening approach, others as an additional stage or as part of a more complex reconstruction. The suitable technique is determined after a consultation and assessment of the anatomical situation.

Who the surgeries are performed for

The surgeries are performed on men over 18 — both residents of Russia and patients from other countries. Patients travel for consultations and surgery from the USA, the United Kingdom, the Netherlands, Switzerland, Bulgaria, Romania, Iraq, and Brazil. Indications and preparation

Medical indications relate to objective anatomical or functional disorders; aesthetic ones relate to the patient’s wish to change appearance or size. In both cases the decision on surgery is made together with the doctor.

General preparation includes a standard set of tests, giving up smoking and alcohol several days before the surgery, and limiting physical activity. Exact recommendations are given by the doctor at the consultation — an in-person consultation before surgery is mandatory: the anatomy is assessed, the approach is decided, and individual recommendations are given.

Can it be combined with thickening

The lengthening technique can be combined with placement of a silicone implant to increase girth — within a single procedure or in separate stages. The decision is made by the surgeon at the consultation.
